The form needs to be filled out for ALL students who will be riding the bus this year.
For students who rode the bus last year but will not be riding this year, parents/guardians need to complete the form and say NO to the question about "Will their bus stop be the same?" and then on the next question select "No transportation needed."
Parents/guardians do not fill out the form if their children will not be riding the bus this year AND did not ride the bus last year. |
